The unfortunate individual with this potent Weakness is carrying an explosive device on the inside. Not easily detectable by standard diagnotic tools and likely (99%) to detonate if tampered with, this device may be remotely activated by an outside agency. This is essentially a lethal leash.
This character is remarkably unlucky and critical failures are much more common.
Condemned to eternal torment, this character is a vampire.
There is something wrong with the design or implementation of the artificial systems of a character with this Weakness and they will tend to fail, sometimes dramatically.
Devourers are condemned to consume the life force of other beings in order to survive.
Sometimes the onboard AI is so potent, it completely subsumes the personality of its host. While not necessarily life-threatening, the often unusual "thought processes" of AI's make this situation apparent to anyone with experience in dealing with Artificial Intellgences.
A Faulty AI is somehow damaged. It will show a strong tendency to behave in unpredictable ways. For the character with this Weakness, it would be very much like sharing one's skull with a raving lunatic.

This character's artificial systems differ enough from known technologies as to make the character dependant upon the original engineer. Attempts at repair and retrofitting are made at greatly enhanced difficulty and spare parts are nonexistant save from the original engineer.
Whether this speaks to artificial systems possessed by a character or that character's own physiology, there is a fair chance that the character may explode.
